The following Symptom Assessment and medical/psychological history items (Patient Portal)  MUST BE COMPLETED ONLINE BEFORE AN APPOINTMENT CAN BE SCHEDULED FOR EVALUATION AS A NEW PATIENT.

yes, it's that important. yes, we need to be able to review this information before your appointment. Yes, it needs to be done online, so it goes directly into your chart. It cannot be done on paper. However for now you will be able to access these assessments only within 10 days of your appointment. NJ will send you instructions at that time.  If you have any  difficulty opening the items, or completing these steps please call NJ at 425 454-8684 so she can help you. 

You do need to get through the entire Symptom Assessment before it will Save; it won't take long.



Our records and Intake system is an electronic format, so this information is entered directly into your records. All of this information is necessary for us to provide you the best service. It cannot be done on paper.
